Gift cards can be purchased for $120 for new patient visits and $90 for follow-up visits. If you are not sure if the person you are purchasing a gift card for is a current or new patient, purchase the $120 option and any remaining amount will go towards a second visit. Gift cards expire 1 year from purchase date. Refunds/transfers are not available. Please make separate purchases for separate gift cards.
